Care Management Solutions

The Care Management Solutions team serves as an experienced guide and resource for older adults and their caregivers. Our experienced care managers partner with clients as they and their families move through the transitions experienced later in life. Services are comprehensive and home-based, ranging from one-time consultations to enduring relationships lasting many years. CMS will ASSESS your situation, ADVISE what services may be helpful and ASSIST with coordination of services.

Consultation and Assessment

If you or someone you know needs support with the challenges that are a part of late life, it can be hard to know where to turn. Obtaining a consultation or assessment from a care manager is a good place to start.

On-going Care Management

An ongoing relationship with a care manager ensures support is there when changes occur.

Counseling

The changes that come with the aging process can be tough for older adults as well as their loved ones. CMS has licensed clinical social workers on staff to provide counseling during these difficult times. Counseling services are now offered in the Denver Metro area and Northern Colorado.

Family and Caregiver Support

CMS care managers are experienced in caregiver education, counseling for family members and caregivers, and conflict mediation.

African American Caregiver Support Program

On-going support groups provide education and peer support to caregiver members of metro Denver’s African American community.

Volunteer Care Teams

Be a meaningful part of someone’s life while adding meaning to your own. Volunteer to be a member of a care team that provides practical and social support to an isolated older adult.
